I am confirming that my child has my permission and the ability to participate in these basketball sessions.  I have discussed appropriate use and conduct with my child and assume full and sole responsibility for my child’s actions while at Monte Fitness Center (MFC), including damage and injury.
I understand that activities like basketball drills, basketball games and general exercise can be dangerous and participation may result in my child suffering minor and major injuries.  I realize that MFC cannot eliminate all risks of injuries on the premises and that therefore I must assume the risk of injury of my child.  Furthermore, when people get together in groups and exercise there will always be bacteria, viruses and other health threats which cannot be completely eliminated by MFC. 
With these disclosures in mind, I agree to fully assume the responsibility for the above described risks and other risks that I may not anticipate but may occur upon the premises and hereby release and hold harmless MFC, its owners, its employees, all basketball clinic instructors and any other person or entity who may be liable by virtue of their relationship to MFC.  Consistent with this assumption and release, I waive any claim or demands based on or account of any injury (physical or mental) or death or property damage, including theft, sustained by my child whether caused by the negligence of MFC or otherwise.  Examples of released claims would include but not be limited to alleged improper maintenance of equipment, grounds or facilities; alleged negligence in instruction or supervision during training or inadequate security or staffing; slipping or tripping anywhere about the club or being assaulted by any person, not an Owner or Staff, while on or about the premises.  I expressly agree that the foregoing release and waiver of liability and indemnification agreement is intended to be as broad and inclusive as permitted by the laws of the State of Washington and that if any portion of this release is held invalid then the balance shall continue in full force and effect.  I understand that by signing this waiver and release I am giving up valuable rights.
